Beyond the Basics: Managing Chronic Conditions with Clinical Precision

When Home Care Requires a Higher Level of Clinical Support

For many families, bringing a loved one home from the hospital is a welcome step forward. But when that loved one is living with a chronic condition, coming home does not always mean that the risk has passed: While they may have medications and a care plan in place, conditions such as congestive heart failure, Parkinson's, or Alzheimer's often require more attention than basic home care can provide.

Small changes can become serious setbacks when they are not recognized early. That is the moment when families realize they need more than help-they need experienced clinical oversight. Vital Bridge Wellness was created for exactly that reason, to fill the space between hospital-caliber oversight and everyday life at home.

Vital Bridge Wellness' nurses are trained to do more than basic task management. With backgrounds in emergency departments, ICUs, and Level 1 trauma centers, our nurses help to manage chronic conditions, such as high blood pressure, diabetes, thyroid issues, a-fib, and beyond. They are also experienced in dealing with complex comorbidities and can recognize changes early, escalating concerns on a timely basis.

Congestive Heart Failure: Minimizing Risk with Early Warning Management

Take, for example, congestive heart failure, a long-term condition in which a person's heart is unable to pump enough blood to the rest of the body. Less experienced nurses might dismiss increased wheezing or fluid retention in the legs (otherwise known as "edema") as regular / expected symptoms of the condition, but Vital Bridge Wellness' nurses are trained more deeply and can recognize when those symptoms are critical early warning signs that need to be shared with the medical team. Our nurses help patients and caregivers understand what swelling or other changes may mean and when questions about diet, hydration, or medications should be discussed with the physician.

Proactive clinical support can also include medication oversight, such as recognizing when symptoms may warrant a physician review of medications, including diuretics such as Lasix. By coordinating with Primary Care Physicians to facilitate necessary dosage adjustments, Vital Bridge Wellness' nurses help to reduce the likelihood of preventable hospitalizations.

Beyond warning signs and medication management, we also provide a human touch to our delivery of education. We want your loved ones to feel in control through self-care steps they can take to manage their condition. While family members might have difficulty convincing a loved one to use compression socks, our nurses can act as a mediator in encouraging this behavior, ultimately taking some of the pressure off family.

Dementia & Alzheimer's: Care Beyond the Diagnosis

Chronic conditions involving memory loss are often difficult and complicated, requiring more oversight than a family member may be able to comfortably handle. Caring for these conditions is not limited to medication reminders; it can extend into how a home is set up and how daily conversations are held.

Outside of medication management, our nurses provide environmental safety checks of loved ones' homes, such as conducting professional assessments of throw rugs, grab bars, and other household safety concerns, to help reduce fall risk and improve safety at home. Our nurses are also familiar with wheelchair transfer techniques and are willing to educate caregivers on those techniques, allowing loved ones to remain safe, yet also more mobile.

Providing support to caregivers is just as important to our nurses as ensuring the highest quality of individualized and skilled care for patients. It can be painful to watch a loved one struggle with memory loss, and it is often difficult to know what to say. Our nurses can help navigate these moments with insight into what to say (or what not to say), as well as passing on re-direction techniques to help keep loved ones happy and less agitated. The transition of a loved one from assisted living to memory care can be hard, and we are here to support the emotional adjustment for both the patient and the caregiver during that transition.

Redefining Quality of Life: Skilled Care is More Than Just Medicine

Condition management is not our sole focus: Being a pillar of support for caregivers and a source of connection and happiness for patients are both equally important to our nurses. One wonderful example of this is a patient with memory loss that Cathy Lunsford, CEO of Vital Bridge Wellness, cares for. Understanding that medication is not the only avenue to help manage a patient's condition, Cathy took the time to get to really know the patient, discovering her vibrant past as a Harley-Davidson rider. To better engage with her, Cathy took the patient to a Harley-Davidson dealership, giving her the opportunity to sit on the bikes and re-live some of her lifelong experiences. Although she might have forgotten the visit ten minutes later, the happiness that outing brought to the patient-in the moment-was a significant benefit to her well-being.

Independence is another important part of well-being for patients. One patient of Vital Bridge Wellness who has diabetes, chronic kidney disease, and congestive heart failure and is in his 60's moved into assisted living. Under the close care of our team, in a few months, he was able to walk 100 feet and is looking for an apartment near his family. With the skilled oversight of our private nurses, stability and improvement were possible for him, and they could be possible for your loved ones, too.

Cathy's approach in Vital Bridge Wellness is different: It's the intentional application of skilled nursing that looks for ways to not restrict life, but to safely maximize it and bring joy to patients as a result. Through this approach, our patients and their caregivers can feel more supported, knowing that an experience clinical professional is watching closely.

The Skilled Advantage: Care and Advice You Can Trust

During her years as an ICU nurse, Cathy saw what can happen when a patient is sent home from the hospital with insufficient education or oversight. Vital Bridge Wellness is designed to help families better understand and manage a loved one's condition while delivering hospital-level clinical judgment and oversight in the comfort of the patient's home. We want caregivers to feel like they can focus more on being a family member again. And we want our patients to know they are in good hands with our nurses' ICU-level clinical knowledge, confidence, and authority.

Chronic conditions don't have to mean the loss of independence. Instead, with Vital Bridge Wellness, patients can help maintain stability through the management of their condition with clinical oversight and human connections that bring joy and improved well-being.
